Are there any questions that you think should be asked or improvements in this survey?
If so, what are they?

I think this application is good, not too short and not too long.
Maybe defining empathy circles more in the description. I only went to one class and could not remember all the details yet.
Do you feel ready to take on role of empathy trainer yet? What will it take?
My question is: Does it always work?
What should I do if I do not have requirements yet? Fill out the form anyway for now and change it later or fill it out later? I am not sure. I don't want to miss out on a spot.

How do you gain your support

No.
I believe that before you even get to the application, that you should tell the applicants that they have to attend empathy circles before they fill out the application.

The dates above are from another course. Anyway, I'll be able to take part in all cohort 9 courses.
none, it is a very good questionnaire
I'd feel much better if this form was made using a self-hosted solution such as Nextcloud or LimeSurvey (XR and Framasoft have them running, so there's no work to do, just use what they'd surely happily offer and I'd love to help with that). Instead of a giant c0mpany trying to centralize power via proprietary centralized web services and that doesn't protect my privacy and everyone else's and abuses the access it has to people's info
The dates are not for the future. Please correct them

What are one to three ways that you see yourself being able to contribute or share the awareness of CoE concepts and practices that would help the world right now? Perhaps organizations that could benefit from learning these simple yet powerful ways to communicate more authentically, to individuals in conflict or those who find themselves in the mediator role and are open to developing new and powerful skills. Of course, donations of time (skills on websites, social media, etc) and financial contributions are also appreciated.

For now, none but would definitely like to commend on this initiative.
There were several signup instructions. One central place would be helpful.
All good
I am a little confused about how this all works (the training). Clarification about the process and dates would be helpful.
What do you think empathy can work to resolve?
None for now.

I have only participated in one empathy circle thus far, but plan to participate in at least 3 before taking the training. I'd like to be able to state my plans for that on this form, especially if stating those plans makes a difference in the possible acceptance of my application. :-)

Yes - 1. Ask about experience with empathy - where, when, how long, maybe even someone's definition of empathy 2. Maybe deliniate the fundamental agreements that you would want participants in this training to agree to here, so that is clear from the beginning.
Don't enough about it yet.

1) Are there a minimum number of training dates that I must participate in? 2) I have not participated in formal empathy circles before but have done a lot of NVC training and facilitation - will that count? 3) How do I participate in an empathy circle via your FB page - I don't see any relevant events besides the facilitator training?
nothing emerges at this time, but will definitely contact you if something surfaces later
Possibly asking for pronouns.

The survey seems good to me!

It might help you as trainers to ask about what people are expecting/want to learn, or what they don't understand/have trouble with when it comes to empathy circles, so you can prepare/correct misguided expectations.
I am hoping the times above are EDT, not Pacific?
COHORT 4
No questions
Possibly ask if the person has in any ways held some sort of empathy or listening circle before...

yes: include the actual time of the sessions: for people in Australia, we would love to join, but often it has been scheduled at 3AM... would be great to have a range of times: a world-wide community
Age should be included. This makes it possible to understand results in terms of demographics.
None come to mind
